The Department of Pediatrics employs nearly 1,000 faculty and staff who work alongside employees of Riley Hospital for Children and students of IU School of Medicine to make Riley Children's Health Indiana's largest and most skilled pediatric system. The IUSM Medicaid Complex Care Coordination Program embeds and coaches nurses in primary care pediatric and adult practices to provide care coordination for children with medical complexity and adults with intellectual disability, as a Indiana Medicaid-funded project with close collaboration with Indiana Title V. The overall goal is to better organize care to improve the healthcare and quality of life for eligible individuals, as well as their families well-being and the cost-effectiveness of their care within a sustainable model.
Job Summary
The Nursing Care Coordinator Coach works as a member of the coaching team to support and train a cohort of nurse care coordinators from primary care practices around the state.
Department-Specific Responsibilities
  • Performs new nurse care coordination orientation, 6 month high intensity didactic training, and oversees learning collaborative cohort.
  • Performs quality improvement auditing and planning with nurses in cohort.
  • Leads case-based problem-solving discussions.
  • Trains new CC's.
  • Provides orientation oversight for each new CC across didactics, observations, and evaluations.
  • Oversees high intensity curriculum - first 6 months to assigned cohort.
  • Schedules didactics and delivers assigned didactics.
  • Delivers weekly one on one coaching.
  • Provides ad hoc consults.
  • Introduces and leads quality improvement activities.
  • Performs monthly chart auditing.
  • Oversees CC learning collaborative cohort.
  • Delivers didactics to build on primary series.
  • Coaches in quality improvement ongoing projects.
  • Provides ad hoc consults.
  • Contributes as member within coaching team.
  • Addresses personal skill building in role of coach.
  • Reviews, improves existing resources, and contributes to development of new resources.
  • Evaluates and reviews teaching tools and processes for ongoing improvement.
  • Shares duties between coaches across cohorts to highlight individual coaches' strengths and support each other in global task completions.
General Responsibilities
  • Collaborates and advises with medical staff for the provision of care for patients. Performs maintenance and programming of applicable equipment. Maintains accurate records of patient's care, condition, and progress. Prepares equipment and assist medical staff during examination and treatment.
  • Coordinates and evaluates educational needs of medical programs. Educates families, caregivers, patients, and schools in the areas of neurological diagnosis, equipment needs, therapies and medications. Develops, reviews, and updates educational materials as needed.
  • Actively participates in quality assessment and improvement activities to improve patient care.
  • Advocates for the patient's educational, accessibility, and equipment-related needs for home, school, and within the community.
  • Demonstrates the clinical expertise to provide care appropriate to the age and development level of patients within medical programs.
  • Schedules therapy sessions, lab tests, and referrals for patients.
  • Works with insurance companies and Medicaid to obtain necessary equipment, therapy, medications, etc.

Qualifications
EDUCATION / WORK EXPERIENCE
Required
  • Bachelor's degree in Nursing with 4 years of clinical nursing experience; OR
  • Associate's degree in Nursing with 7 years of clinical nursing experience; OR
Preferred
  • Master's degree in Nursing with 2 years of clinical nursing experience.
LICENSES AND CERTIFICATES
Required
  • Registered Nurse issued by the State of Indiana upon date of hire.
  • CPR Certificate upon date of hire.

Working Conditions / Demands
This role requires sedentary work as well as extended durations of moving about the workplace. It requires the ability to move objects and equipment weighing up to 25 pounds, and the ability to discern between various objects and devices and to position patients. The person in this role must be able to perform the essential functions with or without an accommodation.
Work Location
Indianapolis, Indiana
This position is eligible to work a hybrid schedule (mix between remote and in-person work), subject to change in the future based on university policy and business needs.
Advertised Salary
$85,000 - $95,000 per year based on experience, skill level, education, and training.
