Nurse Practitioner or Physician Assistant

At CenterWell, you'll not only provide hands-on care inside the exam room but also delve deeper into the social determinants of health, understanding how they impact patient outcomes. Serve 12-15 patients per day, affording you the opportunity to focus more time on individualized patient care
Thrive within a value-based care model, actively improving the quality of life for your patients
Encounter a diverse caseload, spanning from preventative care to managing chronic conditions
Dedicate your time to clinical care, including patient interaction and administrative tasks related to patient care
Your Accomplishments are Highly Valued :
Hold an active, unrestricted NP/PA license in the appropriate state
3-5 years of experience as an NP or PA in geriatrics and/or primary care
Maintain current certification as an NP or PA
Possess or have the ability to obtain a current DEA license
Embrace the role's patient-facing nature and participate in Humana/Senior Bridge's Tuberculosis (TB) screening program if selected

Preferred Qualifications :
Familiarity with the Value-Based Care model
Previous experience in outpatient senior care
