Job Details: This position exists to be part of a case management team and provide client services and follow-up services as needed. The incumbent in this position aids people experiencing personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefit from medical care. This position exists to advocate for patients by securing needed community/financial resources via the application of extensive knowledge of various human resource agencies.QUALIFICATIONS/KNOWLEDGE/SKILLS/ABILITIES
Bilingual Preferred - Hispanic or Burmese
Requires experience in social work field.
Requires transportation to commute to various health center sites; patient homes.
Hospitals and community agencies through various parts of the city.
Requires good written and verbal communication skills.
Requires an understanding of families and poverty and diverse cultures.
Requires extensive knowledge of community resources.
Requires ability to function in multidisciplinary setting.
Requires Bachelor''s degree in social worker or related field.
